Job Overview

Conifer Health Solutions is looking for a Patient Advocate Representative to join our team in Lebanon, PA. In this role, you will assist self-pay patients at the hospital bedside by identifying eligibility for various governmental and non-governmental programs. You will also work with multiple payor sources, including auto insurance, Workers’ Compensation, and private insurance.

Key Responsibilities

✔ Conduct interviews with patients and family members.
✔ Maintain complete and accurate documentation of all account activities.
✔ Perform financial clearance, including collections and financial assistance review.
✔ Follow up on submitted government applications to ensure timely processing.
✔ Build strong relationships with patients to facilitate quick account processing.
✔ Conduct home visits when necessary to assist patients with required documentation.
✔ Communicate case screening results with hospital staff, including case management and social services.

Required Skills & Qualifications

🎯 Knowledge of Federal, State, and County healthcare programs.
💻 Proficiency in Windows, Microsoft Outlook, Excel, and Word.
🗣 Excellent oral and written communication skills.
🔍 Strong attention to detail and ability to multitask efficiently.
🌍 Bilingual candidates (Spanish preferred) are encouraged to apply.

Education & Experience

🎓 High School diploma or equivalent required.
📅 Minimum 2 years experience in Social Services, Hospital Admitting, or related fields.

Physical & Work Environment

🖥 Ability to sit at a computer terminal for extended periods.
🏥 Must be able to walk across hospital facilities, including emergency departments.
🚗 Some travel may be required for home visits.
